For any program MACNC also requires these documents:
1) ACH Draft:
- For Start Up Program
- For Portable Swipe Program
- For all other Programs
2) Photocopies :
- Driver's License or alternate ID with signature
- State Sales Tax Certificate or Business or Occupational License
3) Photographs:
- THE PREMISES- Photograph of the outside of residence
- INVENTORY- Photograph of your booth with inventory on display
4) Voided Checks:
- Two voided checks. They must be original checks, preprinted with account name and address, not canceled checks and not photocopies.
- NOTICE: If your checks are not imprinted with a business name, you must submit two consecutive months of your most recent personal checking account statements
Existing Merchants Only 5) Merchant
Statements:
- Three consecutive months of your most recent bankcard statements.
